Good day!

I just want to ask if what is the process for returning employee to its former government agency?

My appointment is transfer incidental to promotion, turning two months next year na ako sa bago kung agency. I am currently SG8 while on my former is SG5.
But, Im planning to go back to my former agency due to sudden health conditions and also informed them about my plan.

Do I have to reemploy/go thru the hiring process in my former agency? Can you please enlighten me or provide some memo circular to support it?

Yes, you need to undergo the regular hiring process and your former agency should follow the procedures and submission of requirements under their Merit Selection Plan.
