Question Appointment query for skills eligibility Category II

Good day po!

I would like ask for your opinion whether our agency made the right appointment for this scenario which pertains to Category II:
Feb. 28, 2018 - end of one Temporary status with VS for 2 ratings.

March 1, 2018 - he went to csc to apply for category II eligibility, however, the exam division refused to accept his application due to some question on how the agency give rating to the employees which pertains to our SPMS. Since there was some dispute and the employee was unable to obtain the certificate on that day, so the agency issued ANOTHER TEMPORARY APPOINTMENT-reappointment (as if renewal) to the employee for one (1) month effective March 1, 2018 to avoid gap in service. As the days goes by, the dispute was finally settled.

And by...
March 21, 2018- the employee already secured his eligibilty cert. Which also effective on march 21, 2018, so by that time the agency learned that the employee has fully meet the requirements in category II position, the agency issued a PERMANENT APPOINTMENT for the employee effective also on March 21, 2018. Which means within one month the employee received two appointment (temporary & permanent)

The questions are:
1. Does the issuance of two appointments (temporary & permanent) within one month is valid?
2. Our agency was accredited, so what would be the documents needed in RAI if there is that kind of scenario?
3. Do we violate any rules?
4. Or is possible that we should only issued a Permanent Appointment on March 1, 2018 coz it is anticipated and pretty obvious that the employee already fulfill the temporary period with VS ratings its just that there is a delay in securing the cert. due to some circumstances.

Hopefully we can get a reply from you.
